Re-ablement Care is part of a strategy from the Department of Health to assist elderly people who wish to remain in their own homes for as long as possible. After a period of hospitalisation or illness some elderly people require a short stay in a care home where the emphasis can be placed on rehabilitation as well as recuperation.

Our carers, in conjunction with our clients and other concerned professionals, will draw up a plan of care that draws on existing daily living skills. Our aim is to use these skills within the care home to ensure continued mobility and abilities needed at home. We will encourage our clients to be as active as possible and to help themselves, where possible. The plan of care will be designed to suit the individual and will reflect the wishes of the client in terms of the outcomes to be achieved and the time taken to achieve them.

This process is aided by clients knowing that they have help and support on hand for when it is needed. The benefits of staying in a care home are many: the opportunity to practise using existing skills is balanced by being assisted by carers who have knowledge and experience when it comes to providing the highest standards of care.

When it comes to returning home most welcome the 'safety net' provided by our being close at hand; not only can we provide further respite but we are also able to offer 'Home Care' through our registered Home Care Agency.

An integral part of Re-ablement Care is being able to provide consistency in the care we provide. With this in mind care staff working in the care home are able to be a part of the ongoing care deployed in the client’s home environment. If our clients develop a particular relationship with certain staff then it is possible that they can continue to care for the client in their own home. Being able to care in both environments is important in providing Re-ablement care.
